Social Services at Chilton

Hospitalization is stressful, whether you're confronting an illness or welcoming a new baby into your family. Chilton Memorial Hospital's Social Services Department is here to make your experience a little bit easier, providing comfort, guidance and support as you navigate the health care system and cope with the emotional and psychological issues that arise during your stay.

The department is staffed by clinical social workers who have earned master's degrees in their field as well as other licensed professionals with advanced education and training. They offer a broad range of services for patients, families and health practitioners as well as the community, including the following:

  • Discharge planning - ensuring a smooth transition from the hospital to a rehabilitation, long-term care or assisted living facility
  • Psychological assessments, counseling and intervention
  • Referrals to community resources, such as home care firms, area programs and services
  • Patient advocacy - helping patients understand their rights, hospital policies and other aspects of their hospital care
  • Crisis intervention - 24-hour assistance for Emergency Department patients in need of psychiatric and/or chemical dependency evaluation and referrals
  • Bereavement counseling and assistance with end-of-life issues
  • Financial guidance and counseling
  • Educational programs and support groups
  • Community outreach - assisting schools, congregations and other organizations as needed and speaking on various health topics, such as stress management and Advance Directives
