The Diabetes Self-Management Education* Program at Chilton Memorial Hospital helps adults with diabetes take steps that will make them feel better and stay healthier. This program is recognized by the American Diabetes Association for meeting high national standards.

"Program recognition from the American Diabetes Association identifies the hospital as having a staff of knowledgeable health professionals who can provide state-of-the-art education about diabetes self-management for the patients in our community,” said Elizabeth Lesnevich, RD, CDE. Adds Lesnevich, “Program recognition also supports Chilton’s mission to improve the health status of the people that we serve. Obesity and a related increase in Type 2 diabetes in the community attest to the need for this program."

This includes individuals with newly diagnosed or longstanding:

  • Type 1 diabetes
  • Type 2 diabetes
  • Prediabetes
  • Metabolic syndrome
  • Gestational diabetes

  • People who want information on recent advances in diabetes management, such as carbohydrate counting
  • People whose medical therapy has changed
  • Family/significant others who provide support to people with diabetes

  • Individual assessment by Certified Diabetes Educators (a registered dietician and registered nurse)
  • Hands-on group classes where you’ll learn about:
    • diet and meal planning
    • medications
    • importance of physical activity
    • blood glucose monitoring
    • foot and eye care
    • dental hygiene
    • how to avoid complications

A physician’s prescription is required. We will also need your insurance information to verify coverage.
